MSQG Membership Application

The Mountain Stars Quilters Guild (MSQG) operates on a fiscal year basis – July 1st thru June 30th of the following year. Annual membership dues are $30.00 if you join between July 1st and December 31st or $15.00 if you join between January 1st and June 30th.

There are 3 ways to become a member of MSQG:

  1. Download and complete this Membership Application Form then mail it along with your check for the appropriate amount (as noted above) to the address provided on the form.
  2. Come to a meeting where we have forms available for you to complete. You’ll be able to pay in cash or by check. You may also use a debit/credit card but will be assessed a $2 convenience fee.
  3. Join and pay online (will include a $2 convenience fee in addition to your dues).
    THIS IS A MULTI- STEP PROCESS AND ALL THREE (3) OF THE FOLLOWING STEPS MUST BE COMPLETED:
    a)  Fill in the Membership Application Form below.
    b)  Click on the SUBMIT button at the bottom of the form then SEND the form to the Membership Committee from within your email program
    c)  PAY YOUR DUES using the appropriate link below the form.

Note:  Some people using the Firefox browser may receive an error message when they try to submit their form. If you receive that error, you’ll need to join using Option 1 or 2 above.

It is understood that by completing this online application process, photographs taken of your quilts shown at Guild Meetings and/or Retreats can be published on the MSQG website and Facebook pages until specifically denied by you.

  Check here to OPT-OUT of the email service that MSQG uses as a mechanism for telling members about changes affecting meetings and workshops, providing details about local quilt events, and distributing important or time-critical guild related information. The service is called Google Groups and by choosing to OPT-OUT please be aware that you may not receive important notifications such as venue changes or RSVPs for some guild events.

 The MSQG newsletter is automatically emailed to all members in the MSQG Google Groups and is available at the Sign-In table at Business Meetings. However, if you also need a hardcopy for some reason, please check this box. Note: Hardcopies are generally received about a week later than the emailed version.

Clicking the SUBMIT button on the right will open up an email containing your renewal form. YOU MUST CLICK SEND within your email program to send your application to the MSQG Membership Committee.
HOWEVER, YOUR MEMBERSHIP RENEWAL ISN’T COMPLETE UNTIL YOU MAKE YOUR PAYMENT BELOW.
